The renowned Jaslok Hospital & Research Centre recently launched a specialty clinic for expectant mothers carrying twins- TWIN PREGNANCY CARE CLINIC which is 1st of its kind in South Mumbai.

 

The Twin Pregnancy Care Clinic is a specialised Multidisciplinary Clinic providing comprehensive and a dedicated care for such higher order pregnancies to ensure the best possible outcome in twin pregnancies. This team includes Obstetricians specialized in Fetal Medicine, internationally acclaimed Ultrasonologists, Neonatologists and Physicians experienced in obstetric care and supportive services. At Jaslok Hospital, we have all the specialities under one roof. The clinic is run by Dr. Purnima Satoskar and Dr. Shilpa Agrawal, experienced Obstetricians & Gynaecologists with expertise in high risk pregnancies and fetal medicine. Ultrasound is an integral part of twin and higher order pregnancies and it is well-managed at Jaslok Hospital under the care of Dr. Chander Lulla, and Dr. Nitin Chaubal, renowned experts in Obstetric Ultrasound.

The incidence of multiple pregnancies is on the rise because of increasing maternal age and IVF. There are unique challenges in the management of multiple pregnancies. Besides exaggerated pregnancy symptoms like increase in nausea, vomiting, heartburn, constipation the mother can also experience breathing difficulties, increase acidity, backache and leg swelling as the pregnancy grows. This can be managed by medicines, physiotherapy and counselling.

Other risks in twin pregnancy are low haemoglobin (anaemia), high blood pressure (preeclampsia), high blood sugar (gestational diabetes), preterm labour and operative delivery and these medical conditions essentially requires dedicated attention.

Fetal risks are high and complex. Twins are more susceptible to chromosomal problems and birth defects. State-of-the-art ultrasonography and expertise in fetal medicine are extremely important. Counselling and giving a plan of care is an integral part of our Twin Pregnancy Care Clinic

Twins may be born small due to selective Fetal Growth Restriction (FGR) or preterm birth. In identical twins unequal sharing of a single placenta and abnormal blood vessel connections can cause complications like Twin to Twin Transfusion Syndrome (TTTS), Twin Reversed Arterial Perfusion (TRAP), and selective Fetal Growth Restriction (sFGR). These conditions if not detected and managed in time they can cause severe physical and mental handicap to one or both the babies. Special ultrasound guided invasive fetal procedures are often needed which are offered at our twin clinic. The Neonatal Intensive Care Unit (NICU) at Jaslok Hospital is well known for the expert care and higher survival rate of very tiny babies.

The Twin Pregnancy Care Clinic team recently conducted a CME on fetal medicine aspects of twin pregnancy care at Jaslok Hospital in association with Mumbai Obstetrics and Gynaecological Society (MOGS). There were stalwarts from various specialities who highlighted significant issues in multiple pregnancy and management. An interesting panel discussion underlined the management dilemmas in twins. The CME was attended by students, fetal medicine fellows and consultants from various specialities.

 

This dedicated Twin Pregnancy Care Clinic OPD at Jaslok Hospital runs on Mondays and Thursdays between 4pm to 6pm with an aim to provide dedicated and comprehensive care to mothers with twin pregnancies.
